Krachtige Basiszorg (Overvecht Gezond)

Comprehensive primary care approach for disadvantaged neighborhoods, developed in Utrecht Overvecht (~34,000 inhabitants, high burden of accumulated medical and social problems).

01 The context

Core: the 4D-model, an instrument that inventories four domains together with the patient (body, mind, social functioning, environment) and structures collaboration between the medical and social domains, with more consultation time. Overvecht started the program in 2014; since 2019 it was piloted in practices in disadvantaged neighborhoods of Amsterdam, Rotterdam, The Hague and Utrecht, funded by the insurers Zilveren Kruis and CZ and the Achterstandsfondsen; since 2026 it is a structural part of Zilveren Kruis's purchasing policy in the three big cities, and regional networks such as Rijnmond Dokters are replicating it.
