Description

Home-care organization founded in 2006 by nurse Jos de Blok with 4 nurses and 430 euros. It operates with self-managed teams of at most 12 neighborhood nurses, without a management layer, who organize the patient's full care and network. It reports more than 10,000 professionals in some 850 teams (more recent figures cite up to ~14,000 professionals and ~950 teams), with high staff satisfaction and operational savings reported in the literature. The model has been replicated in 24 countries (Sweden, the United Kingdom, Japan, China, the US, etc.).
